March Break Day Camp Program

I agree to the participation of my child/children in Creative Kids Education Centre’s March Break Day Camp Programs.  I hereby discharge Creative Kids Education Centre from any injuries which may arise from the participation of my child in the March Break Day Camp Programs.  In the event of an inability to contact me, I hereby give Creative Kids Education Centre staff permission to seek any necessary medical assistance my child may require while attending the March Break Day Camp Programs
